INSTRUCTIONS:

There are five steps to the player registration process. Please be sure to allow yourself enough time to complete all 5 in one sitting.

Step 2. Athlete Code of Conduct Agreement

Step 3. Parent / Legal Guardian Code of Conduct Agreement

Step 4. Player and Parent / Legal Guardian Contract

Step 5. Fee Payment

Once all the steps have been successfully completed you will receive a copy of your signed documents along with a receipt of payment to the parent / legal guardian email address you provide.

If you cannot fill these forms out digitally please contact us at info@sszsports.com for a downloadable copy that can be sent with a check or money order.

Seasonal dues are $600 per player.
